What this policy covers

This policy covers information collected through this website (smileworkstally.com). It does not cover care you receive in our offices — patient health records are maintained separately and protected in accordance with applicable law, including HIPAA.

Information you share with us

If you use our appointment request form, we collect what you enter: your name, phone number, email address, preferred location, appointment type, preferred time, and anything you add in the message field. We ask that you not include Protected Health Information (PHI) in the form — clinical details are best shared by phone or in person. If you call or email us, we receive the information you choose to provide.

We use this information to respond to your request, schedule and confirm appointments, and communicate with you about your family’s care. We do not sell your information.

Cookies and analytics — what actually runs on this site

We believe in telling you exactly what this website does, so here it is.

This site uses Google Tag Manager to load two tools, and they load only after you choose “Accept” on the consent banner:

• Google Analytics 4 — records the pages you visit, how you arrived, your general location (city level), and your device and browser type. It sets cookies that recognize your browser across visits. We use it to understand which pages are useful and which aren’t.
• Google Ads conversion tracking — records one thing: whether an appointment request form was submitted successfully, so if you arrived from one of our Google ads we know the ad worked. It does not receive what you typed into the form.

Plainly stated: these tools can recognize your browser or device across visits and connect your visit to an ad you clicked. They do not receive your name, and nothing you type into our forms is sent to them.

If you decline the banner, or close it without choosing, neither tool loads and no analytics or advertising cookies are set. The website works exactly the same either way.

No social media pixels, session recording, heat mapping, or chat tracking tools run on this website. If that ever changes, this policy will say so the same day.

Your consent choices

• You can change or withdraw your consent at any time using the Cookie settings link in the footer of every page.
• If your browser sends a Global Privacy Control (GPC) signal, we treat it as declining all non-essential cookies; you can still make an explicit choice at any time through Cookie Settings in the footer.
• You can also block cookies in your browser settings, or install Google’s Analytics opt-out browser add-on.

Third-party services

Parts of this site link to services operated by third parties, which have their own privacy policies:

• Online booking — appointment scheduling is provided by PatientReach; information you enter there goes to them under their policy
• Membership plan signup — provided by Kleer
• Orthodontic bill payment — provided by Moolah; payment information you enter goes directly to them and never touches this website
• Google — reviews links and maps links. Web fonts on this site are served from our own server, not Google’s.
• Social platforms — links to our Facebook, Instagram, and TikTok pages. These are plain links; no tracking code from those platforms runs on this site.

When you follow those links or use those services, the information you provide goes to that provider under their policy, not this one.

Children’s privacy

This website is intended for parents and guardians. We do not knowingly collect personal information from children through this website.
